A 50×50 mm (5×5 cm) photo is 50 × 50 mm (591 × 591 pixels at 300 DPI) on a white or off-white background. The head must measure 34 mm (68% of the height) from chin to crown, which is the part most photo tools ignore — a photo can be exactly the right size and still be rejected for a head that fills too much or too little of the frame.
